Twiggs County Schools is working in partnership with Community Health Care Systems to provide services to the entire Community.
JOB CLASSIFICATION: EXEMPT
JOB SUMMARY:
Under general direction, provides counseling, psychotherapy, consulting, and outreach services for patients of special populations or with health care needs. Participates in testing and emergency services. Assists in planning and implementing patient services and program components. Performs other administrative duties as requested.
JOB RESPONSIBILITIES AND DUTIES:
1. Provides assessment, short-term treatment, crisis intervention and case management to client
caseload and provides clients with consulting and additional referral to community resources.
2. Completes an assessment of patient/family needs with respect to physical, functional, psychosocial, environmental, financial and educational and all other needs.
3. Monitors the care plan to ensure the effectiveness and appropriateness of services; ensure that services are being delivered and meet the needs of the patient.
4. Advises and makes recommendations to physicians, patients and families regarding the availability of resources and coverage, locates the resources, and arranges for the provision of cost effective services for patients.
5. Maintains a database of patients served that includes all support services and efforts made to resolve issues of care.
6. Analyzes psychological aspects of individual interrelationships to gain understanding of an individual’s thoughts, feelings and behavior.
7. Prepares and regularly maintains progress notes, closing, and/or transfer summaries on patients and related duties as required.
8. Demonstrates satisfactory knowledge and clinical judgment to provide appropriate care for clinic patients.
9. Fosters a multi-disciplinary environment, coordinating with primary care providers and psychiatrist as needed, in the format of a primary care behavioral health model.
10. Collaborates with Domestic Violence resources.
11. Coordinates or follows-up on health education resources.
12. Protects the confidentiality of medical records of various media forms (including written, computer generated, audio, video, or other forms of media) as per CHCS policies and HIPAA regulations:
13. Reports actual or potential breaches of confidentiality to appropriate personnel.
14. Performs other duties as requested by supervisors.
KNOWLEDGE, SKILLS, AND ABILITIES: 1. Good interpersonal communication skills. 2. Demonstrates diplomacy with patients/other health care providers/visitors/vendors, etc. 3. MINIMUM QUALIFICATIONS: License by the State of Georgia as a Licenses Clinical Social Worker (LCSE). Three to five years work experience in a behavioral health setting. Knowledge of current trends in the field of mental health. Knowledge of interview techniques.
TYPICAL PHYSICAL DEMANDS AND WORKING CONDITIONS: Good general health. Requires prolonged sitting, some bending, stooping, kneeling, crouching and stretching. Hand-eye coordination and manual dexterity sufficient to operate a keyboard, photocopier, telephone, calculator and other office equipment of the tasks assigned. Requires adequate eyesight. Occasional lifting and carrying of up to 15 lbs. Possible exposure to potentially toxic substances: electrical, flammable, explosive gas hazards; and other conditions common to a clinic environment. Some works areas are small. Noise may be moderate. Requires working under stressful conditions. Requires exposure to unpleasant elements common to health care (illness, emotional distress, trauma, etc.). Is able to perform job functions with proper technique.
